Session notes, is how a therapist maintains their record and keeps the information intact with them. The Session Notes form serves as case notes for each client session. Whenever you see a client or go as a client to the therapist, the therapist maintains a set of papers that includes your history and session notes. 

These session notes contain the information and the techniques that are used in each session, it is like writing all the important things that have happened during the session. Sessions notes need to be carefully maintained so that they can be referred back to whenever needed, also it gives a valuable record.
